**If SERIOUSLY interested in adopting an animal, please fill out the form below. Applications will not be accepted from persons under the age of 18 and WE WILL NOT REPLY TO AN UNDERAGE APPLICANT. A parent or guardian must fill the application out. Note: An application is not a guarantee that you will get the animal you are applying for. Applications will be considered as they come in, and the best home for the animal will be chosen. Judge’s Park reserves the right to reject anyone, at any time. We are located north of Pittsburgh, right over the border with Ohio. Please be aware of where we are located before applying for adoption. Please look at our adoption process page for more information on what is required.

Are you in favor of spaying or neutering pets? If your answer is no, why? *

What vet will you be using? Is that your current vet? Can we contact them for a reference? *

Are you prepared for unexpected vet bills, especially when it comes to exotic pets like rabbits that can be more expensive? *

List any reasons you could foresee ever having to give a pet up to a shelter or give it away (example: bad behavior, destructive, smelly.). *

Have you ever surrendered an animal to the humane society? If so, why? *

Are you in favor of having animals outdoors (domestic animals such as dogs, cats, or rabbits?). Explain why if yes, and what types of animals (not including livestock). *

Do you have any experience with the type of animal you are adopting? If so, what? *

Have you adopted from us previously? *

If yes who, and please explain if you still have them, or if you don't, why. *

Would you be willing to sign a contract stating that if you are unable to care for an adopted animal, you would return it promptly to Judge's Park? *

Would you be willing to give us personal references to vouch for your ability to adopt an animal? If yes, list references here with contact information. *

Please take screenshots prior to submitting as we've been having issues with the form not going through if people are on some systems. You can also send those when you email us.
